The arrows are left in as one of them sees that they are barbed and would be too dangerous to pull them out here without proper equipment to help stem the inevitable blood loss.
While they tend to Eustace, the medics will be insistent on getting Gran looked at, at least to have some healing magic applied along with bandages. If Gran is still stubborn they can allow him on the wagon and will perform his treatment there.
